The Complete Nutrition Questionnaire
If you want a bigger picture,
consider the complete nutrition questionnaire. The cost is $15,
and it provides estimates of about 25 nutrients.

The first step is to find out how you're doing now.
Are you getting too much saturated fat in your diet? Or, maybe

essential nutrients. NutritionQuest's Complete Nutrition Questionnaire
will give you the information you need to make the right choices

The Complete Nutrition Questionnaire is a paper and pencil form
that you can do at home in about half an hour. It asks you how
often you consume around 100 common foods and you just fill
in the bubbles. You send us the form, we perform a computer
analysis and mail (or e-mail) the results back to you. It's
as simple as that. We will soon have the complete form online
as well.

Does it work?
The complete questionnaire (also known as the Block questionnaire)
is widely used by research groups around the country. The validity
of this questionnaire has been tested in numerous scientific
studies. It was found to be comparable to a detailed food record
in identifying people with high fat or low nutrient intakes.
