The Complete Nutrition Questionnaire

If you want a bigger picture, consider the complete nutrition questionnaire. The cost is $15, and it provides estimates of about 25 nutrients.

The Complete Nutrition Questionnaire is a paper and pencil form that you can do at home in about half an hour. It asks you how often you consume around 100 common foods and you just fill in the bubbles. You send us the form, we perform a computer analysis and mail (or e-mail) the results back to you. It's as simple as that. We will soon have the complete form online as well.

What does the nutrition report tell you? Your average intake and the recommended levels of:

  • Macronutrients: calories, fat, saturated fat, monounsaturated fat, polyunsaturated fat, percent of calories from fat, protein, carbohydrates, cholesterol, fiber
  • Antioxidants: Vitamin A, beta-carotene, vitamin C, vitamin E

    Does it work?
    The complete questionnaire (also known as the Block questionnaire) is widely used by research groups around the country.
